How would I code a home visit by family doctor, detailed history adn examination, of highly complex nature? An injection was given to the patient.

99350, established patient home visit (moderate to high complexity). I am assuming that this patient is established, and not a new patient. And the injection, if it was therapeutic, is 96372.

The series of codes for established patients is 99347--99350. They require varying levels of history, exam and MDM--take a look in the CPT book to select the level of service documented and provided.

can the injection be billed separately from the E&M or is it bundled?
