is it appropriate to charge an office visit(99211) when patients come in for protime checks?

All visits must be medically necessary.

If you do the pro-time in the office, get the results, and the nurse discusses with the MD and adjusts the dose, and for Medicare, the service meets the requirements of incident to, you can bill a nurse visit. It must be documented: not just the PTINR and the coumadin dose.

Some consultants feel that if there is no change in dosage, a nurse visit isn't warranted. CMS hasn't been that specific.

we always ask about bleeding and brusing and document answers and do vitals
