Hospice Patient Modifier

I just received a denied claim from medicare saying the patient is enrolled in hospice. The claim has the GV modifier showing that the diagnosis was not related to the admit diagnosis. Does anyone have any clue as for the problems on this claim?

what did you bill- CPT and ICD? Perhaps you billed Hospice care plan oversight and did not link to the hospice-qualifying diagnosis.
