Multiple problems
If a patient goes to her gyn office for a routine pap smear and in addition to the routine visit she also complains of menorrhagia which the doctor treats with meds or possible surgery, can I bill an E&M in addition to the routine pap (ex.99395)?
of course!!
Link the problem coce to an E&M (probably 99213 or 99214) code with -25 and the 99395 to V72.31. Be sure documentation discusses two issues separately.
Preventive and office visit same day
There are a couple of articles in Codapedia that will give you more detail about how and when to do this. Search "preventive" in the search box, and you'll find them.
