PPD testing

We currently bill 86580 with dx: V74.1 when the nurse does the test. When the patient returns it is said that we can bill a 99211. But what dx code do we use? You can not use a V dx code with a 99211. Any suggestions?

no charge for the reading. You get paid for placing it. If it is positive then the patient should see the doctor and there will be decision -making that would qualify for an E&M.
