pre hospital billing
One of my attending ask me to find out how would we bill for prehospital critical care. The go to the scene provide critical care service before patient is move to the hospital, just as an EMS personal would do, but it will be the actual trauma attending physician?

You can bill any site of service when providing critical care. The details and documentation would have to support CC and not just emergency care.

There are questions to be asked in this scenario. Are you part of an ambulance team, etc. Although usually provided in a CCU or ICU, critical care services can be provided in an ED, a non-ICU hospital room, a clinic or a parking lot, etc. Circumstances will dictate billing or not.

the situation was stated that they will be part of the local police department and ambulance team..but i will ask more.and i am sure a pos code will be needed, but what will that be.

our trauma attending will be stated that they would be working with our hospital ambulance team. So here is a senerio..a call comes in man having heart attack in the parking lot of Home Depot..he goes with the hospital ambulance and provide the same critical care services at the scene as if the patient was actually in our hospital trauma bay..
If I got this right the same billing will occur but will have to make sure the POS is 99 = other place of service.
I'm not so sure
first the guy had an MI- that's not trauma; second I cannot see a trauma surgeon riding around like that without some sort of compensation for his time. It is also an issue of malpractice- who is the employer at the time of service? Now once he steps out of the ambulance and into the ED, it is different.
I would suggest you consult your specialty society on this one.

how about the heart attack occur during a MVC or patient was assaulted. As far as the trauma surg being paid for his time.. I am assuming this will fall in line of their responsiblity as being part of the Hospital..
documentation
send documentation, they will ask for it. Is it well documented in medical record? Time must be included as well as critical diagnosis.
