Billing for Aestheticians

When billing for procedures done by an aesthetician, does the supervising dr need to be on site? Can the PA just be on site? Can she perform when neither is here?

Billing for Aestheticians

The Aesthetician is doing extrations, so we bill insurances for acne surgery code 10040. She is licensed to do this in our state in a dermatology office or spa.

Billing for Aestheticians

There are numerous ways this can and can't work, so bear with me. I'm a certified medical manager, certified professional coder - and a licensed aesthetician. (1) If the aesthetician is working as a medical assistant, their services are bundled into the physician's payment from insurance plans and you would follow the health plans' rules for supervision. (2) If the aesthetician is working as an EMPLOYED provider of certain services (and they are W-2'd), and those services can be defined by CPT (i.e. chemical peels) and the services are being performed for a medical reason, then (a) the aesthetician must be working under their own state board's scope of practice, and/or (b) have supervision from the provider whose name is on the claim. The aesthetician must be listed on your liability policy to have supervision pass scrutiny. An aesthetician is not a recognized provider of services described by CPT, which is used universally to report medical services. (3) If the aesthetician is an independent contractor (receives a 1099), you may not delegate supervision to the aesthetician, as they are not your employee to supervise. Be very careful about scope of practice....certain services that seem minor can cause severe permanent damage to a patient. When that happens, the patient will want compensation, and they'll be looking for a malpractice policy. If you employ the aesthetician, you hold the liability insurance - but the aesthetician is still not allowed to operate outside of their licensure. If the aesthetician is an independent contractor, they personally hold their own liability insurance. There are no services that an aesthetician can legally perform (supervised or not) by the Medicare program, because it requires a medical license to be a recognized provider. It gets a little more complicated if the aesthetician is also an RN.......
