pap smear Q0091

I was informed that I should be using Q0091 along with 99394, 5, 6 for a pap smear. I started doing this and every one of them has come back rejected. Do I need a modifier on it???? I'm wondering if I should be using this code at all. Can someone help please....I'm confused!!

In our area, only MCRE will pay with a PE. A few of our payers will pay with an OV (992NN/25). It's best to check with your local payers.
