G0246 for Blue Care Network

Does anyone know what diagnosis code is needed for billing Michigan Blue Care Network when submitting claims for diabetic foot care, G0246. The use of 250.00 does not get covered. I can't find any information anywhere that lists the covered diagnoses.

use codes 250.60, 250.61, 250.62, 250.63, or 357.2. It is only paid if patient has loss of protective sensation, not on every diabetic patient!
