E/M same day different hospitals

We have an infectious disease doctor that saw a patient at one hospital. Then the patient was transferred to another hospital and the doctor saw them on the same day. Can you bill two E/M's for this scenario?

one "visit" per day. If setting changed, hospital to SNF then yes, two visits

Same Doctor, same patient, different hospital?

So Dr. Get Around saw patient 123 at hosptial A and then on that same day patient 123 went to hospital B and randomally saw Dr. Get Around again? Wow. What a coincidence.
