Bicillin C-R

We receive bicillin c-r in 1,200,000 unit syringes. We give children under 30 pounds only 600,000 units and use the rest of the 600,000 units on another patient. I am being told by our local Medicaid (Oklahoma) carrier that I can no longer bill the J0530 for 600,000 units because they don't make 600,000 unit syringes anymore. They are telling me to use the J0540 for 1,200,000 units. Would I bill using a 52 modifier? I am uncomfortable doing this because I have a CPT code for 600,000 units. What would anyone suggest?
