99211 for PPD reading

The article and discussion identify that the reading of the PPD may be billed as a 99211 nurse visit. A large commercial insurer denied the V74.1 dx code with the 99211 as "an E/M code erroneously billed in conjunction with a routine/preventative dx code" and should "rebill using the appropriate preventative medicine CPT code". Did we err in using the V74.1 again for the 99211 reading?

We don't bill it

We consider the reading part of the "global period" for the payment for the placement. Others may disagree but I see no real work in reading a PPD as negative. If it is positive then it becomes a doctor visit.
