how long we should save medical records in paper 7 or 10?

I think that you need to go with what your state says. I know in Wisconsin it is 7 years. The clinic I work at we keep for 10 years.

I believe that CMS requires 6yrs, 3mo; so we keep our 7yrs just to be on the 'safe side' with our 'hard copy' (or "paper") charts. We do not currently have a plan in place for e-records, since the storage issue is in no way the same type of thing as is the case with the paper chart.
As YML3768 identifies, your state administrative rules may identify something altogether different.
