You don't

or you can bill for $0.01. The vaccine is provided free so you can charge the admin fee 90470 with V04.81
Attach a -25 to 99213 (if you want )assuming the patient was seen by the doc for another reason

H1N1 vaccine

You bill for the administration of the vaccine, and an office visit IF the office visit was a separate, distinct service. If the patient came in only for the vaccine, do not bill an office visit. The vaccine was provided free, so as signature doc says, you don't bill for it.

There is an article on Codapedia that shows the admin codes: put H1N1 in the search box. I've copied and pasted some of the article for you:

TWO NEW CODES

G9142 is for the vaccine itself: Influeza A (H1N1) vaccine, any route of administration. Remember, if you receive the serum free, don't bill/report this code.

G9141 Influenza A (H1N1) immunization administration (includes the physician counseling of the patient/family) Report one unit/beneficiary for each administration. Copay and deductible will not apply to code G9141.

CPT® codes

90470—H1N1 immunization administration (intramuscular, intranasal), including counseling when performed
90663—Influenza virus vaccine, pandemic formulation, H1N1

Check with your commercial carriers about how to code: Using CPT® or HCPCS
