36415 Venipunture

Medicare is denying 36415 with an e/m and 85025. My question is, is 36415 bundled with 85025 or do it need a separate diagnosis attached to it? I do not bill it with a mod 25.

You do not need a modifier 25. Can't imagine why it is not paying. Diagnosis should be same as you used for 85025.
