Qualitative Drug Screening

Hello,

I am researching correct usage and units for codes 80100-80104, as well as G0431 and G0434.

Your website has a piece entitled "CPT® Codes 80100-80101, 80102" which states for code 80101 that it is "billable with a count of one for each single drug class method/each drug class, capped at a maximum of 5 units per date of service (some payors, more or less)."

Will you please provide me with sourcing for the cap of 5 units per day for code 80101?

This was just an example of how plans cap based on commercial coverage. "Example" was omitted (sorry).
