Refill diagnoses

When coding diagnoses for a visit for refills for established problems, is it sufficient to code V58.6x (whichever is appropriate, aspirin, anticoagulants, whatever) or should the underlying conditions also be coded?

E.g. V58.69, 250.00, 272.2, 401.1 vs V58.69 alone...

If you are just refilling, then it is not billable as that in itself is not a medically necessary service. The doctor is actually evaluating the problem and making sure the treatment is appropriate so you bill the problem first then if you want add the V58.69.
