Office visit prior to Colonoscopy on Medicare patient

When a patient comes into office to see General Surgeon to set up for a Screening Colonoscopy what is the recommendation for the CPT® code on office vist? Patient is not symptomatic the Colonoscopy is for screening only.

What if the surgeon sends the patient to his primary physician to do a pre-procedure physical? Is this a billable service?
