Nurse Practitioner billing for lipid consultation

I am curious which E/M code a NPP would charge for a 'consultation' about lipid
control and dietary counseling? She goes over the patient's history related to the
lipid control but does not do an exam. She has an assessment and plan in her note. Any ideas? I looked at the preventative codes 99401-99404 but those are
not covered by the major payers.

I would think

that the consultation codes are appropriate with time based coding with a letter to the primary physician outlining recommendations. "Thanks for referring Mr Smith, I recommend xxx diet and xxx medication with follow up after xx months for labs, etc. I have discussed the diet concepts in depth with the patient spending 50 minutes in counseling." This is not preventive- it is treatment of an established disease in the 272 series.

So the NPP can base the level on time, not the History,Exam and MDM? Very good.

Are you clear it's a request from another healthcare professional for an opinion? Not a patient self referring?

I would use the E/M codes that are in the right category (new patient, established patient, outpatient consult.)

Then, if more than half of the visit is spent in discussion, select the level of service based on time. They are in the CPT code, and the typical time is different for each category of service.

Yes, I did check that and verified that the PCP referred the patient for an opinion.
The NP is a certified expert in the field.
