B.M.I coding

I keep getting rejected for when reporting CPT® codes for BMI. According to my records, I am using the correct CPT® and ICD-9 codes with each other. I'm confused. Any suggestions?

I use the appropriate CPT® codes according to the BMI or percentile given as well as the appropriate dx for the correct percentile/bmi. For pediatrics I have been using v65.3, v65.41 and then the appropraite v85.__ code according to the bmi/percentile given by doctor.
