Physician signature on medical records
Does there have to be a signature on the medical record by the rendering provider or is the physicians typed name only enough.

It's my belief that the doctor needs to sign the records for that date of service. His name can be printed, but he/she should sign the record. No stamped signatures.

According to CMS guidelines,
"Signatures are handwritten, electronic, or stamped (stamped
signatures are only permitted in the case of an author with a
physical disability who can provide proof to a CMS contractor of an
inability to sign due to a disability); and
• Signatures are legible."

So electronic signatures/verification are acceptable. So are signature files inserted in a file via jpeg, gif, etc.
