Transitional Care Management

Patient was discharged from short term rehab on 6/18/14 after hip arthroplasty on 5/14/14. On 6/19/14 a nurse from our provider's office returned the patient's call regarding her coumadin/INRs. The provider saw the patient in follow up on 6/27/14 with a note supporting moderate complexity medical decision making and is charging for TCM code 99495.

1. Is discharge from short term rehab a qualifier from TCM?
2. Does the call on 6/19/14 (INR discussion) support communication within 2 business days?
3. Is TCM code 99495 supported?
