critical care

When billing critical care 99291 dos the pt. has at that date at that moment in current organ failure and once the patient is not activly coding or dying but the illness still presist until patient has become more stable billing 99291 is apporiate?..for example some that has Hemorrrhage of gastrointestinal tract and the hemorrhage is somewhat under control, but patient still is as risk for hemorrhage, is this still billable as a 99291?

for that patient if the doc was flushing blood out of the NG, ordering transfusion stat, titrating pressors minute by minute for hypotension, then it is critical care.
