coding for precollege physical

If a 17 year old comes in for completeing precollage admission physicial requirements by his to be college, including reivew of vacinations etc, how do I code that?

Also if they come for completoin of driver's permit physical, which is literraly a review of PMH and, eye color on exam and vision acuity, is their a code for that?

V70.3 with preventive care codes for both school and driver's license (administrative purposes). I personally adjust the charge for the simple ones; I feel bad charging full price for a less than complete preventive visit but that's me. I also take advantage of the visit to talk sex, drugs and life.

Are the documentation requirements difference for a v70.3 or no different than a v70?

Do medicare and commercial payers cover this service, v70.3. What is the appropriate CPT® code: 99395?

Can this v70.3 code be used for if I saw someone who needed administrative paperwork completed with a limited physicial for moving into a assited living facility and CPT® would be a routine e/m for chronic disease.

Medicare - no; commercials- maybe
For a Assisted living, I use the regular E&M and use it as a routine maintenance visit. If they are on no meds and perfectly healthy, then you could use this and charge them, but then they would not need the assisted living facility.
