Billing a hospital vist and stress test in the same day.

How can we bill a physician visit when he sees a patient who is admitted in the hospital, his daily visit and then if the patient has a stress test the same day?
We in the office bill for the visit, and the hospital bills the testing.

The physician is seeing the patient while the patient is an inpatient? If the provider is supervising and doing the interp/report you can bill 93016 and 93018. If just interp/report, then 93018.

Need a 25 modifier on the EM visit, assuming the provider was doing work beyond just the stress test
