Face to Face Encounter for Home Health

One of my doctors just handed me a face to face encounter document for home health. can I bill Medicare for this? If so, what CPT® code do I use?

What did your provider hand you?

There are face-to-face requirements for seeing the patient before/after ordering a HHA service. Did the provider see the patient on this date, or is the provider ordering HHA on a different date than the face-to-face service?
