how do I optimize coding for inpatient admit that conversts to hospice.

When is the best sequence to code this situation. A sick patient is admitted to hospital. 24 hours later, pt gets worse and family elects to make in patient hospice. Per hospital policy, I must discharge patient and readmit (patient doesn't actually leave hospital)

Day 1 -bill 99223 to admit
Day 2 -discharge patient but do not bill and instead bill readmit (99222)with new progress note stating Interval events and see Day 1 note for full HPI, PMH, SH, FH

or

Day 1 -admit 99222
Day 2 - discharge <30
Day 3 - bill admit to inpatient hospice

or
day 1 - bill admit
day 2 - bill both discharge and also bill readmit on same date of service

I would use

choice 2- you do the work of an admission , discharge and readmission so you deserve to get paid for it
