Bundling medical devices with procedure code

UHC has denied (using rebundling as the reason) A4264 medical devices billed with procedure code 58565 surgical procedure to perform an Essure sterilization. Does anyone have any experience with non-payment of the medical devices with this procedure? Any suggestions re where I can research further?
