G0180

does anyone know how to bill for the G0180 on the claim form, do I enter the to and from dates, do I send a copy of the form with the claim? I am so unsure and where do I find the information I need in the medicare manual?

I billed the date I signed it, the diagnosis and place of service- office. Do not send the form or indicate the date range of the home care. Keep a copy of the signed form in the patients chart
