Who is allowed to submit codes to insurance companies?

Who is allowed to submit codes to insurance companies? - If a patient purchases a medical device such as a hearing aid, from a source on the internet, with no hearing exam or consult from an Audiologist, Medical Doctor or Hearing Aid Dispenser, could the patient submit a diagnosis code to their insurance company on their own for the hearing aid purchase? And, if the patient purchased the hearing aid from a Hearing Aid Dispenser, from a facility with no Medical Doctor or Audiologist on staff, can the Hearing Aid Dispenser supply the insurance company (or the patient) with a diagnosis code for hearnig loss/hearing aid fitting, to be supplied to the insurance company?

Anyone can submit to insurance companies!

but whether they pay is a contract matter. In the good old days patients paid the doctor and sent the receipt to the insurer. So if they have coverage for the medical equipment, they can send in the receipt and get reimbursed.
