Destruction of Records Documentation

We are getting to the point in our practice where we really need to get rid of some medical records for space reasons. We have been in practice for 20 years. I know that Wisconsin requires us to keep records for a minimum of 5 years. Just to be on the safe side, we are going to get rid of records that are over 10 years old. Does anyone know what information must be documented when you destroy a record? We have a company that is qualified to do it. I am just not sure what legally needs to be documented upon destruction. Thanks in advance for any help given. I really appreciate it.
