What are the correct CPT® codes for Total Vaginal Hysterectomy with enterocele repair, anterior and posterior Colporrhaphy, Uterosacral Ligament Fixation, and Sling (transobturator) for uterovaginal prolapse (cystocele, rectocele, and enterocele) and urinary incontinence?

Procedure Performed: Total Vaginal Hysterectomy with enterocele repair, anterior and posterior Colporrhaphy, Uterosacral Ligament Fixation, and Sling (transobturator).

Per the details of the Op Report the surgeon completed the Vaginal Hysterectomy (135 grams) including the closing the vaginal cuff AND then performed the anterior and posterior colporrhaphy for the cystocele and the rectocele repair. The Uterosacral Ligament Fixation was then done to reduce the enterocele. The anterior and posterior Colporrhaphy and Uterosacral Ligament Fixation were done via vaginal incision.
