abnormal findig during a annual

If during a routine annual physical exam visit, I come across an abnormality that requires furthur history and diagnostic testing which I perform during the same visit- eg: breast lump or neck mass, whats the best way to claim that revenue.

Can the documentation be including in my physicla exam progress note?

If you inquiring about your ICD-9 codes the answer can be found in the Official ICD9 guidelines which can be found in your book or online.

actually it's not the coding. Can I bill a annual visit as well as a separate E&M code 99213 for the patient according to medicare rules?

Yes to both; Medicare covers it and most commercial insurers do too. Be sure your documentation is separate in the history, assessment and plan sections
