return visit to review abnormal lab

We had a pt that had a physical. Vitamin b and d labs were ordered with dx 780.79. The doctor had the pt return to office to discuss the low readings of these two tests. she coded for b12 deficiency and vitamind deficiency. Shouldn't it have been coded for the 780.79 or for abnormal lab (not sure of code). The note clearly stated pt came in to discuss lab. Please help/

return visit

If you have a known diagnosis, use that. Use signs and symptoms when there isn't an known diagnosis. In this case, since the lab worked confirmed a diagnosis, I'd use that.

What concern do you have about it?

No concern but ins. is denying claim. I wanted to make sure I was doing everything possible.

what is the denial?

I could see them bundling the result review office visit into the initial office visit but that is pretty brutal. What CPT codes did you use?
