opthalmology medicaid guidelines

is there any specific diagnosis that would override Medicaid's frequency limitation? also, if we send in medical records, would that convince medicaid of medical necessity?

It is specific to the state and the diagnosis. But the notes should be very clear why the visit frequency is medically indicated. "Just because" and "I am the doctor" will not work.
