POS - Hospital discharge, nursing home admit

With the new POS rules for Medicare we are not sure what to do. Previously when our doctor had a hospital discharge and a SNF admit on the same day (both done at the hospital) we would do the SNF admit with a POS 31.
What do we do in regards to the new rules? Can we still do the POS as 31, but with the hospital address? HELP!

Are you saying that the SNF is at the same address as the hospital or that the doctor is doing the SNF admit at the hospital and not at the SNF?

He is doing the admit while the patient is still at the hospital. The SNF is not located in the hospital.

Well that is not kosher. He needs to do the admit at the SNF. That needs to be the place of service. Signing papers is not admitting to a SNF.
