Modifier 90
When or where should a modifier 90 be used? Someone mentioned to me that when ordering a TSH (84443) with Thyroxine (84436) and T3 (84479) a modifier 90 should be attached to the latter of the two tests. I do billing for a doctor that has a lab in his office and these tests are done on the premesis.

Mod 90 if a reference lab is running the test
