medicare coding

Our physician saw a patient for medical condition and he also was asked to fill out an Optum healthcare quality patient assessment form.
This took considerable extra time other than just the office visit. Is there any way to bill medicare for this time.

Who asked the doctor to fill out this form? If patient needed it filled out, then the patient should be charged. If the insurance company requested it, then they should be charged, unless its in your contract. If your billing Medicare, then you would use 99080 and have the patient sign a waiver (ABN) that Medicare does not cover this cost.
