64635 and 64636

We are being denied for the following, any input would be appreciated...

64635 - LT
64636 - 59 - LT
64636 - 76 - LT

Its is being denied for "Procedure Code exceeds limit of units billed"

Is your whole claim being denied? or just one of the codes? When billing CPT® 64636 bilaterally you should bill it w/ modifier 50 and price it at one and one half times the original fee. Maybe it was denied because of the LT/RT?
