Ordering vs. Supervising Dr for LAbs

When a patient comes in for a blood draw (36415) do you bill under the Ordering physician or the Supervising provider since the ordering doc was not there that day? According to Medicare this draw does not fall under incident to guidelines, so this is causing an issue between who has to sign off on the nurses note that the pt came in.

I would really appreciate a link or other credible resource.

It is the ordering doctor. Why would anyone have to sign off on a blood draw?
