can a modifier be added to a preventive visit (99385)

It probably could but...

you should add the modifier -25 to the other code 99213, etc. Perhaps one of the experts can explain why the problem code gets the modifier and not the PE, perhaps because the primary reason for the visit is the PE and the problem is the secondary reason.

we are also billing G0101 & Q0091, but is being denied as inclusive to the preventive visit.. can a modifier be added to the CPT® codes G0101 & Q0091????

99385 and Modifier

Was the fee reduced by the charge of the G0101 and the Q0091? I am assuming this is a Medicare patient and the 99385 isn't covered by Medicare, so that charge would be patient responsibility. Since the G0101 is the payable E&M, the modifier would need to be on the 99385.

Are you billing this to a commercial patient? If so, the pelvic and breast exam is part of the age/gender appropriate exam and should not be separately billed or paid. The Q0091 is equivalent to 99000 and some commercials pay it and some don't.

If it is a Medicare patient, you should bill Medicare for the G0101 and Q0091. Bill the patient the difference between your charge for 99385 and your charge to Medicare for the other two services. Look at the example in the article "Preventive medicine services for medicare patients"
