SNF visits for skilled nursing home patients

I am a physician who follows patients in a local nursing home after the patients are discharged from the hospital to a local SNF for rehab or IV antibiotics or other skilled needs. These patients are discharged home upon completion of their skilled needs. How often I can visit these patients.
