? about CT scan coding and contrast billing

I was wondering if the CPT® code 74178( which states CT/abdom w and w/o contrast) includes the charge for the actual contrast. As a provider that bills mostly X ray codes isnt billing for the Contrast the same as me billing for the x ray film or developer....which I cant..its in the Xray code. A family member of mine just got her bill back from the hospital for that code and they billed her 4000!!!!! and that was just the tech component..the professional comp was billed out at 350 which the INS co says she owes 150

Is $4000 for the tech comp of 74178 reasonable and cust
My CPT® book said this code has a total value of(off the top of my head) 12.89 and the tech comp value of 9.3

From those values where does the charge of $4000 come from?

Fantasyland?

BTW I have found that Medicare pays about 350 for that code and Work comp in KY pays 615

Of course it is fantasy No one pays retail but docs and hospitals have to bill a lot for that rare insurer that pays a percent of charges. You have to charge more than your highest reimbursement so as to not leave money on the table. If you have to pay cash, talk to the finance people. Offer to pay Blue Cross or Medicare rate, cash in full. they'll probably take it.
