V codes

I am looking for a spreadsheet with the list of V codes that are acceptable for first listed diagnosis in outpatient settings. There is a list of codes that can only be used as primary/first listed diagnosis in the ICD-9-CM book.

I am confused by the book. Some of the codes listed require additional digits, yet the codes with the additional digits are marked as those to not list as a primary diagnosis.

Any help would be appreciated.
