I need someone to help me to find a reputable website that states that it’s inappropriate not to have the full description with the ICD-9 code in the header history.
We have a doctor in our practice not wanting to change the screens for the Bone Density where they are just sending the ICD-9 code in the header without a description.
As a coder I think that a full description should be documented and not only the icd-9 number.
