CPT codes 45378 & 45380

Can CPT codes 45378 & 45380 be billed together? Insurance compaies are denying these codes as inclusive.

I believe that they should not denied as inclusive. Instead, Multiple endoscopic rule should be applicable?

Cannot bill together. 45378 is included in 45380.
