New 1500 Claim Form

Does anyone know whether service dates prior to 4/1 can go on the old form (even if printed after 4/1)?

Version 02/12 CMS 1500 forms will replace the current CMS 1500 forms, effective with claims received on and after April 1, 2014.

Does not matter what the DOS is.

Good Luck! Ours were on back order for 2 weeks!

Our office was told by several insurances that if the DOS was prior to 04/01/14 the claim could go on the old CMS 1500 paper, but if the DOS was AFTER it had to be on the new form.

CMS 1500 Claim Form Instructions: Revised for Form Version 02/12


Form Version 02/12 will replace the current CMS 1500 claim form, 08/05, effective with claims received on and after April 1, 2014:

• Medicare will begin accepting claims on the revised form, 02/12, on January 6, 2014;
• Medicare will continue to accept claims on the old form, 08/05, through March 31, 2014;
• On April 1, 2014, Medicare will accept paper claims on only the revised CMS 1500 claim form, 02/12; and
• On and after April 1, 2014, Medicare will no longer accept claims on the old CMS 1500 claim form, 08/05.

keywords being EFFECTIVE WITH CLAIMS RECEIVED ON AND AFTER 4/1/14. That is how I read it, anyway. It states nothing about DOS. In fact, I mailed a claim to a commercial carrier on and old form for an OLD dos, and they returned it, stating I needed to use the NEW form.
