Postobstructive Pneumonia

What ICD-9 code should be used for postobstructive pneumonia, the patient also has stage 4 squamous cell lung ca.

I doubt

there is such an ICD even though it is a common term. I would code bacterial pneumonia due to x bacteria/virus or whatever else they found on bronch. Or 482.9- it is specific enough for my needs...

I would assign code 486. I would not assign bacterial pneumonia code based on lab values without the physician confirming that. If you follow coding guidelines/coding clinic never assign a code based on lab values alone. A physician must document bacterial pneumonia or document strep pneumonia or the specific bacteria.
