Medicare denial of 99202-99204 for Not medically necessary

Very recently I have been receiving requests from medicare for a signed ABN and physician documentation for 99202-99204. These codes are being billed from a speciality office, we very rarely have a patient sign an ABN for a office visit. I have submitted the notes for the visit and I am receiving denials for "Not Medically Necessary" The notes being submitted are well documented visits that support the level of service bill.
Is anyone else having this issue?

I would think there is more to it. I would call Medicare and ask. We've been getting incorrect denials because of a system issue they are having which has not been fixed yet. This could possibly fall into one of those categories? I would still call and ask just to make sure.
