Apligraph use with Simple Mastectomy

Because the 19303 is being completed as an outpatient procedure, billing the use of the apligraph skin substitute, used for preparing the site for a Breast Reconstruction, is hitting a medicare edit for Q4101 LCD only applies to diabetic wound care for chronic ulcers. Is there another HCPCS code that should be used when this product is being used during other surgical interventions?

The LCD means it cannot be billed to Medicare- it is not a covered benefit. If you want to use it, the patient should sign an ABN and will have to pay for it. FDA approval does not mean Insurance will cover.
